Case Management Services
Under the Medicaid Aged and Disabled Waiver Program, you are entitled to Case Management services.
The case manager is responsible for your services, health, and safety, linking you to the services you need and developing a service plan to fit your unique needs.
What you can expect from your Case Manager:
Monthly telephone calls
Quarterly home visits
Annual and 6-month assessments
Advocacy
Linkage to community resources
